Using personal information responsibly
At EIDAP Inc., we understand the importance of individual privacy. We know that earning the trust of our clients and employees is a permanent and ongoing endeavor. With the growth of electronic media it is necessary to take further steps in protecting the privacy of the individual. We fully support the new Canadian legislation, the Personal Information Protection and Electronic Documents Act, "the Privacy Act", in its role to protect the privacy of the individual. Our role in protecting an individual's privacy does not stop with this Act. We are continually looking for additional ways to ensure that the personal information we hold is secure, accurate and used in a responsible and respectful manner.
As such, OUR President and Chief Executive Officer is our Chief Privacy Officer and he will be responsible to ensure that the personal information entrusted with us is secure and will be used for the purpose intended and not be used for purposes other than what is necessary to offer the service our clients deserve. When you have your animal microchipped, and if you lose your animal or find an animal, there is certain information that we require to serve you better. This information is necessary in order to identify you and contact you for the purpose of returning the animal to you or its rightful owner at the earliest possible. These may include:
- Name
- Address
- Phone number(s)
- E-mail address
- Animal and veterinary information
To fulfill our mandate and to make your experience with animal ownership more positive, we are developing a number of customized services and programs. To do this, we need all information that pertains to you such as address, phone numbers, alternate contacts, veterinarian name and address and animal description.
Ten Privacy PrinciplesPrivacy Legislation and EIDAP Inc. support the following ten privacy principles:
A. AccountabilityEIDAP Inc.'s Chief Executive Officer will serve as its Chief Privacy Officer.
B. Identifying PurposesWhen personal information is collected by EIDAP Inc., the purpose for its collection will be made clear. If the purpose changes, the organization will obtain "fresh" consent before the information will be used for the new purpose.
C. ConsentThe knowledge and consent of customers and clients is required for the collection, use and disclosure of personal information. The consent of the client is assumed when the animal is microchipped and registered in the Eidap Animal Registry (EAR)
D. Limiting CollectionPersonal information will only be collected by EIDAP Inc. for the purposes that were identified at the time of collection.
E. Limiting Use, Disclosure and RetentionThe personal information collected will not be used or disclosed for purposes other than for what it was collected: the return of the animal to its rightful owner.
F. AccuracyEIDAP Inc. will endeavor to ensure that the information it collects is accurate in all regards.
G. SafeguardsPersonal information will be protected, by appropriate security safeguards, against loss, theft, unauthorized access, copying etc. including maintaining files in locked drawers and offices, the use of computer passwords, etc.
H. OpennessA full set of policies and procedures relating to privacy and the management of personal information are to be readily available to individuals upon request.
I. Individual AccessClients or customers of EIDAP Inc., who wish to view the records pertaining to them in the EAR database, may do so by contacting the Company's Chief Privacy Officer. Clients or customers may obtain a copy of their record. A client or customer may provide a written request to the Chief Privacy Officer for correction related to any data contained in their own file or record. Response to an individual's request should be made within 30 days.
J. Challenging ComplianceComplaints regarding this policy or any privacy issue should be directed in writing to the Chief Privacy Officer at EIDAP Inc. 14 Chippewa Rd. Sherwood Park, AB, T8A 3Y1 (info@eidap.com) Upon receipt of a complaint, the Chief Privacy Officer will:
- Acknowledge receipt of the complaint within 72 hours.
- Collect the facts through reviewing the relevant record(s) and interviewing any staff members involved.
- Within 14 days, respond to the complaint describing the action to be taken to correct the situation or the reasons why no action is to be taken. All responses to complaints shall include the following: "If you are dissatisfied with the outcome of this investigation, please contact the Chief Privacy Officer for further review."
Complaints regarding this policy should be directed in writing to the Chief Privacy Officer of EIDAP Inc.
